Subacute care is comprehensive inpatient care designed for someone who has had an acute illness, injury or exacerbation of a disease process. Subacute care is generally more intensive than traditional nursing care and less than acute care. It requires frequent (daily to weekly) patient assessment and review of the clinical course.
Our comprehensive program is for patients in need of short-term recovery after a hospital stay as a result of an illness or injury. The goal is to maximize independence and function. Subacute services are designed to return patients home safely and as soon as possible, with most patients being discharged after 20 days.
Therapy programs are initiated after an evaluation and are delivered according to a physician approved treatment plan.
Every room in our subacute unit is a private room. A physical therapy gym in this unit is reserved for our subacute patients.
Our Rehabilitation components consist of Physical, Occupational, and Speech Therapies, and activities of daily living training.
