  Provides information and referral on programs and services for individuals with disabilities.
  
  Provides case management services to those who are unable to access services on their own behalf.
  
  Coordinates the Personal Assistance Services Program (PASP) serving individuals with disabilities who are employed, attending school or volunteering in the community.

  Assists disabled adults to remain at home and at the highest level of functioning as long as possible.
  
  Avoids premature institutionalization in hospitals or nursing homes due to unmet health and home care needs.
  
  Provides assessment, care planning, service coordination, monitoring and reassessment in the home.
  
  Medicaid Waiver assistance is also available through statewide programs such as Jersey Assistance for Community Caregiving (JACC), Community Care for the Elderly and Disabled (CCPED) and Caregiver Assistance Program (CAP).
